Genetic factors contribute to the pathogenesis of coronary artery disease (CAD). We studied 100 patients with CAD and 50 healthy individuals to assess the association of endothelial nitric oxide (eNOS) polymorphism (Glu298Asp) and angiotensinogen polymorphisms (M235T) and CAD in an Egyptian population. Serum nitric oxide (NO) and angiotensin I levels were also measured. The frequency of Glu298Asp and M235T polymorphisms were higher in the CAD group compared with controls. The mean level of NO was significantly lower (P < .05) while angiotensin I was significantly higher (P < .05) in patients CAD than in controls. The frequency of eNOS TT allele of M235T variant was significantly higher in patients with CAD (20% vs 6%). The frequency of angiotensinogen (AGT) TT and T allele in patients with CAD was significantly higher (P < .05) than in controls (22% vs 6%and 47% vs23%, respectively). Homozygosity for Glu298Asp and M235T polymorphisms may predispose to CAD.

{In addition to the immaturity of air sacs and surfactant deficiency, preterm infants with respiratory distress syndrome (RDS) have abnormalities in the pulmonary vascular bed. We aimed to study two known modulators of pulmonary vessels: endothelin-1 (ET-1) and L-arginine. We hypothesized that plasma concentrations of ET-1 and L-arginine could correlate with the severity of RDS. We prospectively studied 71 preterm infants (gestational age = 29 to 35 weeks) with and without RDS. We measured plasma ET-1 by enzyme-linked immunosorbent assay and L-arginine by spectrophotometry. Infants who continued to require oxygen support or died by day of life 28 were considered to have bronchopulmonary dysplasia (BPD). ET-1 concentrations were significantly higher in RDS infants ( P = 0.039) than controls. Among infants with RDS, it was significantly higher in those who later developed BPD ( P = 0.026). L-arginine was significantly lower in RDS infants ( P = 0.001), but did not differ between BPD and non-BPD infants ( P = 0.19). There was no correlation between L-arginine and ET-1 (R = 0.1

Epidermoid cyst of the external auditory canal (EAC) is rarely encountered in the clinical practice, but when it occurs, it may cause obstruction of the meatus that necessitates surgical excision. The aims of this study were to present 9 pediatric patients with epidermoid cysts of the EAC and to evaluate the outcome of the surgical technique that has been used in excision. Surgical removal of the cyst was carried out through a simple transmeatal approach, a medially based rectangular skin flap was elevated and the cyst was completely removed. No complications or recurrence have been reported.

Malignant Pleural Mesothelioma (MPM) is nearly invariably lethal tumor of the pleura. Significant therapeutic nihilism exists among health professionals. Recent progress has reshaped the clinical landscape in the treatment of MPM. Two European guidelines have been published, one from the Task force of the European Respiratory Society (ERS) and the European Society of Thoracic Surgery (ESTS) and the other from the European Society of Medical Oncology (ESMO). With these guidelines and recommendations as a guidepost, this review discusses the major changes and their impact on the management of MPM.

Introduction: Thermal injury of the hand is characterized by disfigurement and deformity with marked emotional problems because the patient is no longer able to perform the daily living activities and function at school or work. Early excision and grafting (E&G) were introduced to decrease hospital stay, hospital cost, and septic complications and to eliminate burn toxins. In this study, E&G was compared with delayed skin grafting in deep hand burns. Materials and Method: A 40 patients with deep second- and third- degree hand burns with average burn size less than 30% total body surface area (TBSA). Patients were randomly divided into E&G group and delayed grafting group. All hands in both groups were subjected to pre and post operative program of physiotherapy. Measurement of total active motion (TAM) of each digit and grip strength was recorded pre and post operative. Hand function using Jebsen-Taylor Hand Function Test (JTHFT) were recorded three months postoperative in both groups. Results: There were statistically significant differences in both groups regarding to TAM, hand grip strength and Jebsen-Taylor Hand Function Test and in favoring to E&G group. Conclusion: The study concluded that early excision and skin grafting with physiotherapy gave better results than delayed grafting in terms of preservation of hand function and short hospital stay.

Objective: To retrospectively evaluate the reliability of offline manipulation of archived three-dimensional (3D) ultrasound volumes in the assessment of the normal fetal brain and the diagnosis of fetal brain abnormalities. Methods: Seventy-nine 3D volume datasets, archived at the time of scanning, from women attending a tertiary center, were analyzed. They comprised 52 cases with brain abnormalities and 27 normal controls. Postnatal magnetic resonance imaging or postmortem examination confirmed the final diagnosis in all cases with brain anomalies. Offline analysis of the 79 anonymized 3D volume datasets was carried out by three fetal medicine experts, examiner one (E1), examiner two (E2) and examiner three (E3), using 4DView software. The experts were blinded to any prior diagnosis or history. Data were collected on a specially designed data sheet and entered into a specialized database for analysis. Results were compared between examiners, with the initial two dimensional (2D) diagnoses and with the final definitive diagnosis by calculation of percentage agreement and kappa coefficients.

Micro RNAs regulate posttranscriptional gene expression and have been recently identified to regulate cellular differentiation, establishing immunological tolerance and are involved in the pathogenesis of several diseases. miR-181-a, expressed in haematopoietic cell lineage, has shown to be an important modulator of B and T cell differentiation, maturation and function. This study aims to identify the expression signature of miR-181-a in the peripheral blood of paediatric SLE and its regulatory effect on the consequent expression of its target gene PCAF.
