Melilotus indicus, is a traditional medicine used as analgesic and emollient. Although Melilotus indicus extract (MIE) has recently been shown to suppress growth of several tumor cell lines, information regarding its antitumor mechanism is completely unknown. Here, we report the mechanism underlying the effects of MIE on human hepatocellular carcinoma cells, specifically HepG2, and SNU-182 cells. Methanolic MIE impaired the proliferation, and induced cell death in both HepG2 and SNU-182 cells but not in normal hepatic L-02 cells. Mechanistically, flow cytometric analysis revealed that MIE induces apoptosis in HepG2, and SNU-182 cells. However, MIE-induced apoptosis were not affected by a pan caspase inhibitor z-VAD-fmk as well as MIE did not stimulate caspase activation. Furthermore we found that MIE-induced apoptosis could be attributed to a mechanism involving mitochondria-mediated pathways evidenced by decrease in the mitochondrial membrane potential (ΔΨm), increase in the Bax/Bcl-2 ratio, and translocation of apoptosis inducing factor (AIF) from the mitochondria to the nucleus. Suppression in AIF expression by siRNA reduced MIE-induced apoptosis which suggested the dependency of MIE on AIF to induce apoptosis in hepatocellular carcinoma cells. To the best of our knowledge this is the first report elucidating the anticancer mechanism of MIE. Our findings suggested that MIE might be a good extract for developing anticancer drugs for human hepatocellular carcinoma treatment.

BACKGROUND: Patients with transient ischemic attack (TIA) are generally clinically unstable, with fear of developing a handicapping stroke. Identification of those at highest and lowest risk of stroke in the first days and weeks after a TIA would allow appropriate use of worthy secondary prevention strategies.

OBJECTIVE: Incorporation of a clinical scoring system, neurovascular imaging, and magnetic resonance-diffusion-weighted imaging (MR-DWI) to help predicting risk of developing an ischemic stroke following a TIA.

SUBJECTS AND METHODS: A prospective observational study was conducted on 25 patients with TIAs, 64% were females, and 26% were males, with a mean age of 57±10.36. Patients were assessed clinically and an ABCD(2) score was applied. Patients have undergone diffusion-weighted imaging (DWI), within 24 h from the event, and intra- and extracranial duplex study. Patients were followed up at intervals of one week, three months, six months, and one year.

RESULTS: Six patients (24%) developed stroke on their follow-up, most of them (83.3%) had their strokes within the first three months and had an initial ABCD(2) score of ≥4. The development of stroke was associated with the presence of significant extra and/or intracranial vessel disease (P=0.006) and the presence of acute lesions on their DWI (P=0.035).

CONCLUSION: Incorporation of brain MR-DWIs and neurovascular imaging together with the ABCD(2) score improves prediction of ischemic stroke following TIA.

Aim of the Study :Hypospadias repair is one of the commonest operations done in pediatric surgery centers, with an incidence of 1 in 200 to 1 in 300. The proximal type accounts for 10-15% of cases. In this study we describe the results of the Koyanagi Nanamura procedure, which combines a meatal-based flap and a pellicle island flap in a single procedure. It allows for excision of ventral midline chordee without jeopardizing the flap. The technique was modified to improve the blood supply to the neourethral flaps The modified technique was found to achieve nearly normal phallic cosmetic appearance with a low complication rate.

Methods: This study was conducted at the general pediatric surgery unit on 20 cases with proximal hypospadias operated by modified Koyanagi technique.

Main Results: Twenty cases were operated; with mean age was 2.83 ± 1.17 years. Sixteen cases (80%) were penoscrotal, 4 cases (20%) were perinea'. Six cases (30%) had associated external genitalia anomaly. Twelve cases (60%) received topical androgen for a mean period of 179 ± 0.66 months. Mean operative time was 193.5 ± 4184 minutes. The follow up period ranged from 2 to 12 months, with mean of 8.15 ±3.53 months. The overall success was fulfilled in 11 cases (55%), as they needed no reoperation. Early complications were bleeding (5%), urine retention (5%), infection (5%) and stenosis (15%) which responded to urethral dilatation. Late complications included penoscrotal fistulae (20%), meatal recession (25%), urethral diverticulum (5%) and torsion of the glans (5%).

Conclusion: A single staged repair can be safely and effectively performed even in patients with the most severe proximal hypospadias. Modified Koyanagi repair performed for severe hypospadias with chordee gives a good cosmetic and functional result.

In this paper we present a fully integrated metamaterial-based phase shifter. This design replaces the printed TLs used to obtain the low-pass response with their lumped L-C equivalent circuit. This allows the integration of the phase shifter on single MMIC, and results in a large area saving. Furthermore, the proposed phase shifter achieves a low return loss across the entire tuning range although it does not use any active inductor circuits as in our previous designs. Hence this design is completely passive with no DC power consumption, and is expected to have better linearity and noise performance.
