Hydroxychloroquine niosomes: A new trend in topical management of oral lichen planus
A b s t r a c t
The work aimed at studying a novel topical niosomal gel formulation of hydroxychloroquine for the management of oral lichen planus. Niosomes have been reported as conceivable vesicles to deliver drug molecules to the desired mucous membrane or skin layers. Hydroxychloroquine niosomes were designed using different methods of preparation. Tween 20 and cholesterol in molar ratio (1:0.5) were used. The prepared systems were characterized for entrapment efficiency, particle size and in vitro drug release. Different factors affecting the encapsulation of hydroxychloroquine in niosomes were studied vs. varying the type of surfactant, the cholesterol:surfactant molar ratio and the amount of the drug. The selected noisome formulation was dispersed in different gel formulations and evaluated according to the in vitro drug release and the physical stability. The results showed that the type of surfactant, cholesterol ratio and incorporated amount of drug altered the entrapment efficiency and the in vitro release of hydroxychloro-quine from niosomes. The optimum formulation was prepared by reverse phase evaporation technique using Brij 98:cholesterol molar ratio (1:1.5) and containing 20 mg of hydroxychloroquine and incorpo-rated in 20% w/v Pluronic F-127 gel. A double-blind, controlled clinical study was performed using two groups of patients. Group A (n = 11) who received hydroxychloroquine niosomal gel formulation, one application-a-day over 4 months showed 64.28% reduction in the size of lesions and the average score of pain was reduced from “4” to “1”. Compared to placebo group B (n = 5), who showed only 3.94% reduction in the lesion size and the average score of pain was remained “3”. Our results suggest that these niosomal formulations could constitute a promising approach for the topical treatment of oral lichen planus in short time with less side effects and no recurrence after stopping the treatment.

The nuclear orphan receptor (Nurr1) has recently received a perceivable solicitude as a target for the therapeutic intervention against PD. Meanwhile, the dysregulation of autophagy, along with other processes is believed to contribute massively to PD pathophysiology. Hydroxychloroquine, a hydroxy derivative of chloroquine, is an antimalarial agent which is also used as an anti-rheumatic drug. The neuroprotective potential of hydroxychloroquine and chloroquine remained controversial until recently a study showed that chloroquine exhibited an antiparkinsonian activity through Nurr1 modulation. The aim of this work is to identify whether the less toxic derivative, hydroxychloroquine, could show a similar pattern. In rat rotenone model, hydroxychloroquine effectively boosted Nurr-1 expression, exhibited an anti-inflammatory effect as verified by hindering certain pro-inflammatory cytokines and successfully reduced GSK-3β activity. Consequently, an increase in the striatal tyrosine hydroxylase content, as well as improved locomotion and muscle coordination was shown. However, this improvement was opposed by hydroxychloroquine induced autophagic inhibition as manifested by enhancing both LC3-II and P62 levels possibly through the prominent decline in sirtuin 1 level and elevated apoptotic biomarkers. In conclusion, hydroxychloroquine successfully ameliorated PD motor dysfunction in spite of the fact that both autophagy and apoptosis were deregulated through Nurr1 modulation.

The Food and Drug Administration has recently approved the off-label use of hydroxychloroquine (HCQ) for the treatment of coronavirus disease 2019 (COVID-19) infections. However, a recent study not only failed to demonstrate HCQ efficacy but also documented a serious side effect of COVID-19 therapy with HCQ: QT prolongation and secondary arrhythmia. HCQ has been used as an off-label drug and deemed safe and effective for the treatment of oral lesions, such as Sjogren syndrome (SS), chronic ulcerative stomatitis (CUS), and oral lichen planus (OLP). Because HCQ may be appropriately used for the off-label treatment of SS, CUS, and OLP, relevant safety concerns regarding HCQ therapy with regard to dosage, drug-to-drug interactions, and QT prolongation and secondary arrhythmia are discussed here. Because of the possibility of decreased pharmacy supplies of HCQ, replacement drugs with respect to patients with SS, CUS, and OLP being successfully treated with HCQ are also discussed.

AbstractAn analysis of multirecess hydrostatic bearings with linear flow-pressure supply characteristics is presented. These characteristics can be obtained using either a constant supply pressure source with a laminar flow restrictor before each recess, or, as suggested in this paper, a multiport viscous pump. Analysis of the viscous pump-bearing system is made for odd numbers of recesses, and it is shown that a staggered or skip-one sequence of connection of the pump ports to the bearing recesses gives the least unbalanced reaction between the pump rotor/stator. The flow-pressure characteristics of viscous pumps with peripheral shrouded Rayleigh steps in a cylindrical configuration are derived and used to design a five-port pump to match a hydrostatic bearing. An experimental pump is designed, manufactured and tested, and the results obtained at one port show good agreement with the theoretically calculated values of pressure and flow. If this port is taken as representative of a pump, which is manufactured with higher accuracy, then the principle of having an effective multiport viscous pump is hereby validated.
