BACKGROUND AND STUDY AIM: Haemophagocytic lymphohistiocytosis (HLH) is a life-threatening clinical syndrome with liver involvement varying from mild dysfunction to severe fulminant failure. The aim of this study was to present a case series of four HLH patients presenting with acute liver failure (ALF) in the neonatal period.

PATIENTS AND METHODS: All four patients were neonates at the onset of symptoms. They presented to Cairo University Pediatric Hospital with ALF; they underwent prompt investigations including determination of ferritin, fibrinogen, and triglyceride levels as part of our ALF workup. Further investigations were tailored according to the associated clinical features and the results of preliminary investigations.

RESULTS: HLH was diagnosed according to HLH-2004 criteria. Three patients fulfilled at least five out of eight criteria. Fever, splenomegaly, elevated ferritin levels, and low fibrinogen levels were present in all patients. The fourth patient had a serum ferritin level >10,000ng/ml, favouring the diagnosis of HLH, despite fulfilling only four out of eight criteria. For three patients, positive consanguinity and previous sibling death were reported, suggesting a genetic aetiology of HLH.

CONCLUSION: ALF can be the presenting feature of HLH; thus, a high index of suspicion is necessary. Fever is a hallmark, especially in neonates. Diagnosis is important for this potentially treatable condition.

Haemodynamic effects of sodium taurocholate (S.T.) were studied on isolated guinea pig's auricles, rabbit's heart, rabbit's aortic strip, guinea pig's tracheal chain as well as the blood pressure and ECG pattern changes in pentobarbital anaesthetized dogs. S.T. induced significant negative inotropic and chronotropic effects on the isolated auricles of guinea pig's especially in higher concentrations. Using isolated rabbit's heart, the negative inotropic and chronotropic effects induced by S.T. were found to be depending on the concentration. Cardio-inhibitory actions of the salt are not due to either cholinergic beta 1-adrenergic blocking effect or nicotine like activity. S.T. in all tested concentrations had no effect on the contractile response of isolated rabbit's aortic strip or guinea pig's tracheal chain and did not prevent the contractile response induced by noradrenaline and histamine. In anaesthetized dogs, i.v. injections of the salt in a dose of 30 mg/kg b. wt. produced a significant decrease in systolic and diastolic pressure, but lower doses induced no significant changes. A dose of 30 mg/kg b. wt. of the salt potentiates the decrease in systolic and diastolic pressure when coadministered with the neuromuscular blocking agent, atracurium besylate. Atropine, propranolol and phentolamine did not alter the hypotensive effect of S.T. (neither cholinergic nor beta 1-adrenergic blocking effect). The electrocardiographic pattern induced by S.T. (20-30 mg/kg b. wt.) in dogs were mainly characterized by decrease in heart rate and prolongation of P-T interval.

Two groups of buffalo calves were infected with Sarcocystis fusiformis sporocysts. Animals of the first group received each 5 X 10(5) sporocysts, those of the second group 5 million sporocysts. All calves were clinically normal during 6 weeks after infection. Minor changes were observed in the blood cytology, serum alkaline phosphatase, glutamic oxalacetic transaminase, glutamic pyruvic transaminase, total proteins, urea and glucose of infected buffalo calves.

We present the SUSY QCD corrections to the production of light neutral MSSM Higgs boson with a pair of supersymmetric scalar partners of the top quark at next-to-leading order at LHC. We study the dependence of the lowest order total cross section on both the renormalization/factorization scale and the Higgs mass in the framework of minimal supergravity, taking the point SPS1a as a benchmark, for the numerical calculations. The dependence of the total cross section is strong. Including the NLO SUSY QCD corrections significantly reduce and stabilize the dependence of the lowest order total cross section on the renormalization/factorization scale.
