The low price and high contents of bioactive compounds in citrus peel increase interest in using it in various applications. Mandarin (Citrus reticluata) peel belongs to Rutaceae family and is rich with antioxidants. However, limited studies are available on toxicity of Mandarin peel oil (MPO) on human Prostate Cancer (PC3) cells. Therefore, the present study was conducted to study the cytotoxicity and genotoxicity of MPO on Human normal Fibroblast (HFB4) and PC3 cell lines. The half maximal inhibitory (IC50) and safe concentration of MPO was detected using MTT assay. Comet and DNA fragmentation assays were performed to assess apoptotic DNA damage. Also, the ROS level was evaluated and the mRNA expression level of apoptotic and antiapoptotic genes were measured using RT-PCR. Results of the cytotoxic test showed that MPO induced preferential inhibition of PC3 cells proliferation in a concentration-dependent manner with IC50 10.97 µg/ml. The time-dependent induction of DNA breaks demonstrated in PC3 cells treated with MPO safe concentration-stimulated ROS generation and apoptotic DNA damage through increased expression of tumor suppressor p53 and Bax genes and decreased expression of Bcl2 and MDM2 genes. In contrast, non-significant changes were observed in the DNA integrity, ROS levels and expressions of the tested genes in the normal HFB4 cells treated with MPO. Thus, we concluded that MPO induced preferential cytotoxic and genotoxic effects toward cancerous PC3 with no noticeable toxic effects in normal HFB4 cells and therefore further in vivo studies are recommended to test its possible use as anticancer drugs.

Background: Patients with advanced cancer experience distressing symptoms and progressive decline in their performance status (PS) as death approaches.
Aim: To identify the relationship between symptom burden and PS of Egyptian cancer patients receiving palliative care.
Methods: This was a prospective observational study that included 100 patients with advanced cancer. Symptom burden was assessed using the Arabic version of the revised Edmonton Symptom Assessment System (ESAS-r). Performance status was assessed using the Palliative Performance Scale (PPS).
Results: The mean total ESAS-r score was 60.1 (±10.7). The most common symptom to be reported as severe was pain (93%) followed by tiredness (74%), poor wellbeing (67%), lack of appetite (62%), anxiety (60%) and drowsiness (56%). The majority of patients (76%) had a PPS score ≤30 with an overall mean PPS score of 36.2 (±13.3). There was a statistically significant negative correlation between the total ESAS score and the PPS score (r = - 0.687, p = < 0.001).
Conclusion: The results suggest that Egyptian patients with advanced cancer experience high symptom burden and significant decline in PS. The higher the symptom burden, the poorer the PS of patients with advanced cancer.

Pathogenesis of thrombocytopenia is heterogeneous and resistance to treatment is a great challenge. In this study, we reviewed the demographic features of thrombocytopenic Egyptian patients. We also analyzed the role of T cell in chronic immune cases. IL-12, IL-35, IL-17, and TGF-β were measured by ELISA. The median age at the time of diagnosis was 30 years and its range was 14–70. The median platelet count at the time of diagnosis was 15 × 10⁹/L. Regarding treatment and follow-up, there was an indication for treatment in 96% of patients. Of the 150 ITP patients who were given first-line therapy (corticosteroid 1 mg/kg/day PO), there was a complete response (CR) in 40.3% while 59.7% patients were nonresponsive to therapy. Forty-five chronic cases fulfilled the criteria for cytokine assay. Comparison between the case group and the control group revealed statistically significant lower platelet count in cases, while the four measured cytokines were statistically significant higher in cases rather than the control. Correlation between the platelet count and the level of cytokines was statistically insignificant. The remission rate in ITP on steroid as first-line therapy is less than 50%. The higher expression of IL-12 and IL-35in chronic ITP is due to the persistently higher Th1 activity which explains the continuity of the disease, while the higher expression of Treg cytokines (IL-17 and TGF-β) may be explained by effect of immunosuppression or upregulation of the receptors on Treg cells.
