Abstract:

Background: Acne scar correction remains a challenge to the dermatologic surgeon. Non ablative laser treatment has been shown to affect significant dermal collagen remodeling with minimal post treatment sequelae.
Objective: To compare the efficacy and safety of a 585 nm pulsed dye laser (PDL) and a combined PDL and 1064 nm Nd:YAG laser for the treatment of acne scars.
Methods: Thirty patients with mild to moderate atrophic acne scars received five PDL and combined PDL and Nd:YAG laser treatment sessions at three to five week-intervals. Treatments were administered randomly in a split-face manner. Patients were photographed before treatment and 3 months after the last treatment session. Histological evaluations were performed on skin biopsies obtained before treatment and 3 months after the last session. Collagen quantification per area, before and after the treatment, was performed by morphometry, with computerized image analysis. Patient satisfaction and clinical improvement were assessed using standard grading scales.
Results: Both treatment modalities induced noticeable clinical and histological improvement with significantly better results in combined PDL/Nd:YAG laser versus PDL alone. Patient satisfaction scores concurred with these improvements. The study demonstrated a positive significant correlation between the duration of acne scars and the degree of scar improvement.
Conclusion: Both lasers are effective and safe modalities in the treatment of atrophic acne scars with significantly higher scar response to the combined PDL/Nd:YAG treatment modality. Patients with shorter duration of acne scars (<2 years) respond better than those with longer duration.
