PURPOSE: The purpose of this study was to assess the impact of testosterone therapy on mood and cognitive symptoms in perimenopausal and postmenopausal women.

METHODS: A retrospective cohort study undertaken in a UK specialist menopause clinic. 510 women using hormone replacement therapy (HRT) with persistent low libido, cognitive and negative mood symptoms were treated with testosterone cream or gel for 4 months. A modified version of the Greene Climacteric Scale was used to measure self-reported symptom frequency and severity at baseline and 4 months after initiating treatment.

RESULTS: All nine cognitive and mood symptoms significantly improved across the study period. Mood improved more than cognition (47% of women reported an improvement in mood vs. 39% reported an improvement in cognition; 34% vs. 22% decrease in mean symptom scores, respectively). Regarding libido, 52% of women reported an improvement; mean symptom score decreased by 33%.

CONCLUSION: Transdermal testosterone therapy for 4 months was associated with significant improvements in mood and cognition. Further research including randomised clinical trials are needed to establish the long-term efficacy and safety of testosterone for the treatment of menopausal cognitive and psychological symptoms.

BACKGROUND: The high rates of the sustained virologic response 12 weeks after treatment (SVR12) in real world settings provoked the adoption of shortened courses of the costly direct-acting antivirals (DAAs) regimens. This study provides, to our knowledge, the first systematic review and meta-analysis for the efficacy of the shortened 8-week course of sofosbuvir (SOF) plus daclatasvir (DCV), the most accessible DAAs in the low-middle income countries (LMICs).

METHODS: We performed a proportion meta-analysis to determine a reliable rate of SVR12 by pooling all studies that evaluated the results of the 8-week regimen of DCV + SOF. In addition, we applied sensitivity analyses using two imputation paradigms: a conservative approach, and a pragmatic approach to avoid overestimating the efficacy of the 8-week regimen in studies that followed a response-guided treatment (RGT) approach.

RESULTS: Six studies with a total of 159 patients were included. The pooled SVR12 rate ranged from 91 to 97% in the included scenarios. The pragmatic scenario showed that the pooled SVR12 was 97% (95% confidence interval (CI) 91%; 100%) with lower variability as assessed by the prediction interval. The conservative approach revealed an SVR12 of 93% (95% CI 84%; 95%).

CONCLUSION: The 8-week course of 60 mg DCV with SOF provided a comparable SVR12 to the standard 12-week regimen in treatment-naïve, non-HIV co-infected patients with a minimum estimated efficacy of 90%.

BACKGROUND: The number of people living with dementia and/or cognitive impairment worldwide is rising with a negative effect on quality of life for many older adults. This study aims to examine the factors associated with cognitive function among older adults in Kuwait.

METHODS: This cross-sectional study recruited 253 older adults ≥ 60 years from a Geriatric outpatient unit in Kuwait. Cognitive function (dependent variable) was assessed using the Arabic version of the Mini-Mental State Examination (MMSE) with scores < 24 indicative of cognitive impairment. Biochemical, nutritional, clinical, lifestyle, anthropometric, and sociodemographic independent variables were included.

RESULTS: A normal MMSE score was reported for 51.0% (n = 129) of the sample, with 34.7% and 14.2% of participants having mild and moderate/severe cognitive impairment, respectively. Multivariate ordinal logistic regression analysis indicated that Type 2 diabetes was associated with more than double the odds of cognitive impairment (OR = 2.15, 95% CI: 1.19-3.94; P = 0.01). Each additional level of education was associated with a lower likelihood of cognitive impairment (OR = 0.34, 95% CI: 0.26-0.43; P < 0.001).

CONCLUSION: This study identifies key risk factors associated with cognitive impairment in older Kuwaiti adults. These findings underscore the need for targeted interventions to mitigate cognitive decline in aging populations and provide context-specific data to support policy decisions.

Liver fibrosis presents a unique diagnostic challenge in people coinfected with hepatitis C virus (HCV) and human immune deficiency virus (HIV). This study aimed to explore the association between CD4 discordance and liver fibrosis in that population, alongside assessing the predictive power of different models for significant fibrosis. A cross-sectional study was conducted on 198 adult people with HIV/HCV coinfection. Liver fibrosis was noninvasively assessed using transient elastography, and CD4 discordance was defined based on the discrepancy between absolute CD4 cell count and CD4 cell percentage. Multivariate logistic regression and receiver operating characteristic curves were used for analysis. Only 52 (26.3%) individuals had concordant CD4 values. The study found a significant correlation between high CD4 discordance and significant liver fibrosis ( < .001), with a higher prevalence of significant fibrosis in those with high discordance (65.5%) than those with low (14.5%) or concordant (13.5%) CD4 values. High CD4 discordance was strongly associated with significant fibrosis (odds ratio = 11.48, < .001). The CD4-only model showed a high negative predictive value (87.5%), making it suitable for excluding significant fibrosis. In contrast, models incorporating both CD4 count and percentage demonstrated higher positive and negative predictive values (78.6% and 87.6%, respectively), indicating their utility in diagnosing significant fibrosis. This study highlights the complexity of assessing liver fibrosis in HIV/HCV-coinfected individuals and underscores the value of CD4 discordance as a predictive factor. The predictive models, especially those combining CD4 count and percentage, provide an approach for evaluating liver fibrosis. Further research is needed to refine these models and enhance their clinical applicability.
